These are info I found by searching RFD. I could only find one in Mississauga. Please refer to the links and call them to verify if the costs mentioned are the same.

Note on this from rfd: He sat in front of the 2nd Cup with a small lineup of people with documents in their hands. Everyone gets their 2 min at his little lattee table to sign the doc and to get the seal stamp after "swearing" that the document was filled out truthfully. Just call during his business hour and arrange an appointment at the 2nd cup near Spadina / Bloor.

Yes, Pravin Kalara has this service available where he is working. The officer is available few days only so (RBO should) check by calling them.
