medical request received without forms

Hi please advice. My friend received medical request from the case officer but the forms are not included with letter. What should she do? Is it okay to book an appointment without forms? Does she has to request forms from case officer?
Please advice asap.

The forms that the CHC provides to you are numbered and are case specific, which means that only you can use the form with that number and it gets locked in to your FILEs (number.)

So call for them, and then WAIT.

You have lots of time.

Hi ,

I hope you can help my friend with this query. By mistake my friend forgot to include this information(student visa rejected in 2006) in her PR application 2010. It was an honest mistake. Now Case manager asked her to clarify this. How should she go about that. He actually waived her interview but asked her to provide this clarification in order to process futher.

Please provide your input.

I am not on the same wave length as you are with your friend. Provide us with all of the information to tell you what it is that you should do to rectify the situation.

NOW, did the friend get her medicals done without the forms?

Or she did not receive any forms but completed her medical and then forgot to include this information?

Or totally ignored the request and submitted the application and now falling back on the fact that she did not receive any (specific request and NO) Forms to go with that request?

It might call for a new application and with a medical completed to go with it. But we will wait for the information from you, that which your friend received as specified in the rejection letter.

Sorry for the confusion.
she applied in 2006 for student visa and got rejected. After that she moved to New Zeland on Student visa. Now she is apply for PR.

She applied in Jan 2010 and submitted documents. Her online status shows "in process". Yesterday She received a letter from her case officer indicating her interview is waived and in order to proceed further she should undergo for medical, pay landing fee and send required documents. These documents are for the clarification of not indicating that she was refused for student visa in her PR application . She just received letter without medical forms.

I hope this is clear.

Now you know. Send the required fees and the documents to the Immigration Officer who is handling her files. He also knows that she was refused a Student Visa.

Did she get her Police Clearance from NZ. Provide all of the documents called for and if she has stated that her request for a Student Visa to Canada was refused, then it is there for him to see and understand that there are other countries that do provide a Student Visa and people do get educated elsewhere and then want to come down here and settle down.

After the receipt of the fees she will be asked to proceed to a location close by and get her Medicals done, and together with it she will receive the forms to complete the Medicals. (Look for the numbers on the right hand top corner.)

She made it to this level. Good luck with the medicals.
