My cousin was born in Canada in 1998. His birth-parents immigrated to Canada and are citizens of Canada.
At an age of 3 years he was adopted by his by-birth-father's brother who lives in India. Since then he lives with his adopted parents in India and has not visited Canada.
Since last 4 years my cousin's birth-parents, in Canada, are not claiming any child support for my cousin.

Now, we would like to change the "parent"'s name in my cousin's passport - Canadian passport.

My cousin's adopted-parents would like to have their names in all official documents. - documents like my cousin's passport, his birth certificate (which is canada issued).

How can this be achieved ? Any help, links, reference or direction would be appreciated.
