I have two questions.
1. My sister has two different name one is before marriage with father middle name and one after marriage in which family name changed and no middle name My confusion what the status I will select Smt/Mrs. Or Kumari/Ms in from line number 1 and 3 respectively? As I copy and cut below. As per rules both status must be same.
2. I will ask my friend in India to make demand draft of Rs971/- and send it to me here in favor “NSDL-PAN” Mumbai but I am confused what I have to write in payment details of demand draft where he asked for drawn on ______________ bank, payable at Mumbai? Is it bank name from where we made demand draft?

1) The NEW NRI Pan Card should have her Married name. SO, she is a MRS or Smt.
2) You are making a lot of work and mailing etc., for NOTHING. Try to minimize the costs and mailing delays and extra time it takes. It is only $16 u.s or $18 or so Canadian. Use your Credit Card.
Drawn on.... : Means, which bank you bought the DD from.
Drawer.... is YOU. And you get it from your Bank which is where you 'DRAW' from, means pull it out of... which Bank. " DRAWN ON"
Most of these Banks have a Central Clearing House, that is where it gets EXCHANGED for CASH. ALL THE BANKS BRING IN THESE "INSTRUMENTS" and EXCHANGE THEM FOR CASH.
Currently they all are done Electronically with the Magnetic ink Charecter Recognition System. (The numbers that they have at the bottom of the Cheque or the DD. that which permits them to do the transaction.
Also things have advanced further and currently they use the photo features in your mobile phone system to take a picture and deposit it into their account. OR scan and Cash.
If the money is paid for a Pan Card for Mrs/Smt x y z, then the DD should be drawn on behalf of Mrs./Smt x y z.
The New Pan Card will be issued to her, with the Name and details in her passport with the Married Surname. ( The sender's Name)
But they will ask for the proof of her married name and the Maiden Name. (Nee' -- Naitre) Born with.

I will fill up online 49AA as per your first link.
If I understand correctly if you have residence and office address in Canada you can’t pay by credit card. One of my relative is coming to Canada so he will bring demand draft. So I don’t have to pay any shipping charge.
If I put her married name in LINE 1 of 49AA form, I have to select same for LINE 3 of 49AA form which is wrong (that’s bug me). We must have select same status in LINE 1 and LINE 3 as per their rules. Strange!!!
Still I am confuse for what I put “drawn on ________________bank, payable at Mumbai”
You said two different things!!!
1.Drawn on.... : Means, which bank you bought the DD from.
2. If the money is paid for a Pan Card for Mrs/Smt x y z, then the DD should be drawn on behalf of Mrs./Smt x y z.
I think number 1 is correct answer because it looks like asking bank name.
I didn’t see any note/instruction saying we have submit proof of her married name and maiden name. It means they will ask later. I just fill up 49AA form with fake names (because not ready for DD yet, 15 days limit) and it turn out when we print filled form didn't saw name before marriage.

I don't want to confuse you more than you want to.
In line 3 they ask, "Are you known by anyother name", It is YES, she was born with a name. It is known as NEE' i.e., Born with. Which is her MAIDEN name. Here it will be KUMARI. If she has any documents, such as School Leaving Certificate, or a Birth Certificate, it will substantiate it. (Some had passports too, that is also a proof of.)
For Payments, if you are a resident Abroad, then you can pay by a DD, or by a Credit or a Debit Card. Only Three accepted modes.
DRAWN ON is used in conjunction with a Bank. Drawn on behalf of means the person who is buying it or some one who is purchasing it for another individual. What is confusing is the word DRAW!.
They 'tally' the DD with an individual and the statement there in the application. Which nails it down to ONE individual and BANK from which s/he bought it from.
You have understood it correctly.
As regards the line 1 and line 3, you must read it a few more times to understand.

I understand what you said for DRAWN ON. I will put Bank name where I purchase Demand Draft.
LINE 1 and LINE 3 I understand correctly and what you said is correct too.
But when you filled up ONLINE form you can't select Mrs. for LINE 1 and KUMARI for LINE 3.
You can select ONLY one status for both LINE 1 and LINE 3 which is either Mrs. or KUMARI.
Just try to fill up online 49AA form up to line 3 with fake names.
AS I said earlier I try with fake names and it turn out LINE3 will not appear in final print out. So I will go with Mrs. in both LINE 1 and LINE3 .
EDIT ::: you have to filled up complete forms to see error message for line 1 and line 3 must be same status.
Please say if I am wrong.
I am only reading the forms and instructions given in their do's and don'ts. What actually happens on their website when you actually fill in with the details you have at hand, could be entirely a different scenario. No comments there. Please check all of the instruction materials at each location, given by them for filling the form 49AA
