PIO application questions for kid

I am in the process of applying for PIO for our kid born here, please share answers to these questions (Parents are holding PR)
(I did search the charcha but did not get these answers)

14. Whether the applicant or either of his/her parents or grand parents or great grand parents
held an Indian passport at any time, if so details thereof:
(a) Passport Number :_____________________(b) Place of Issue :_____________________
(b) Date of Issue: ________________________(d) Date of expiry:_____________________
(Enclose original and photocopies of relevant pages)

*Do I need to give both parent's India passort details?
* What is meant by "relevant pages" :cuss:

15.Whether the applicant or either of his/her parents or any of his/her grand parents or great grand parents was born and was permanently resident in territories falling in present day India, Pakistan, and Bangladesh as on 30.3.1999 and if so the following details may be indicated:
(i) Date & Place of birth :________________________________________________
(ii) Proof of residence etc :________________________________________________
(Enclose photocopy of relevant documents)
(Note: If applicant is the spouse of a citizen of India or a person of Indian origin, his/her particulars should be indicated)

* No idea what to fill here? what photocopy?


& What all doc's do I need to get notarized?

Relevant pages means the Bio page - page 1 and 2 as well as the last pages which talk about your parents names etc. You can provide only ur and ur spouse's ppt pages not grand parents...

Question 15 talks about Pak and bangladesh and if you had no one there on that date, you don't have to fill that out.

Any photocopy you make HAS to be notarized.

Hi

I applied for my daugters PIO card a few weeks back and received the card last week. I applied through Toronto consulate office.

I submitted plain photocopies of the passport, PR Card and landing document. I was not required to notarize any of the copies. The officer verified all the copies with the originals and that was it

For Q.14 of yours, give complete details of your passport as well as your wife's passport. The place provided is short so I had to adjust my writing to accomodate my passport details as well as my wife's pssport details

Relevant pages would be first and last page of Indian passport alongwith other pages which have got an immigration stamp on it as well as the ECNR stamp

I left Q.15 blank and that was accepted

Thanks guptamik & incanada, I would like to apply by mail (instead fo going in person and waste my off hours!) what do you suggest?

hi desis, it is already 3 weeks since i submitted the pio application for my daughter at Indian consulate toronto, till now I have not received the same, what do I do now? how long to wait? I tried calling the consulate as usual some recorded message keeps playing...

Did you submit by mail, as you stated in on of your previous messages? If you submitted in person, did you tell them that you want to pick it up or did you provide a prepaid envelope for them to mail the card to you? When I applied for my child's PIO card, it was ready for pickup within a week!!
