You know the forms that you filled. You also know the area that you 'failed to fill'. Please go through the forms and get another set of forms ready to submit after you receive the confirmation. After you receive it, you will have a FILE number to which you can address your responses to. So, hang in there till you receive it.

After you have the FILE NUMBER allocated to you, you can send in the next set of forms with the corrections and point out to them what you missed and say you were tardy in you submission. Please submit the same with an a letter of apology.
