dnageena, the OHIP personnel can ask for copies of your passport and thus determine if and when you went to India. It is not a big deal for them to verify this.

I was honest enough tell the OHIP people that I am working in US and my employer given the tickets. They wrote 2 reasons in my report that my kid born in US and I have originated travel to india from US.

But I have proofs for more than 2 years that I am living in windsor. I cross the border everyday, bank statements, 4 credit cards, utility bills, canadian car, car insurance with canadian gmac, rental agreement, ontario driving license and what not.

Last month OHIP responded back that they need my social acitivity proof. (letter from church) Mostly I travel to toronto on business trip. I collected letters from my clients, the restaurant I usually eat, windsor friends, toronto friends....i sent responded 2 months back. I got a letter that they are reviewing my case.

I have very solid proofs (I really living here in windsor) that I lived in ontario for more than 1200 days. Do I have to wait for OHIP card to come so that I can add that as a second picture id (first picture id is my driving license)for my citizenship or I can go ahead with my Driving License and Passport?

if you have stayed in canada for more than 3 years then you should be ok .
about the picture id ( what about your PR Card ) i guess that should do as an id .

I have stayed more than three years but they need two picture id's apart from PR card. (atleast i believe so). I can use my driver's license and passport but i am just wondering will it raise any unnecesary question if use passport as sencond piece of id instead ohip card?

You can use your Ontario driver's license and passport as proof. They will be checking your passport anyway when you go to write citizenship test.

You can call CIC call center or take help of organizations like YMCA if they conduct citizenship classes if you have any doubt regarding which documents to attach.
