Thanks RBO. In my case, the name change has already happened for my wife. But it was done not on the Indian passport, rather done later when she got CDN citizenship (really easy process, she just had to show her marriage certificate). Now since the names are different on the two documents, they need some sort of a name change affidavit.
I'm wondering what the format of this should be, but will look online for samples.
Here is a form that you can use.
SWORN AFFIDAVIT
By this deed, I, the undersigned (new name) _______________________________, son of
________________________________, formerly/lately called (old name) _____________________________, working as/doing (give profession/occupation)_____________________________, permanent resident of _____________________________ and presently residing at* _______________________ and holding a Canadian passport no. _________________, issued at _________________, on _________________, do hereby solemnly affirm and declare :

That for and on behalf of myself and my wife and children and remitter issue, I wholly renounce/relinquish and abandon the use of my former name/surname (old name) ______________________ so that I and my wife and children and remitter issue be hereafter called, known and distinguished not by my former name/surname but assumed name/surname of (new name) ___________________________
2. That for the purpose of evidencing such determination, I declare that I shall at all times hereafter, in all records, deeds and writings and in all proceedings, dealings and transactions, private as well as official, business and all others and upon all occasions whatsoever, use and sign the name of (new name) ________________________ as my name/surname in place of and in substitution for my former name/surname.
