Most grateful , if you could guide me on: the Applications forms required to apply under the skilled worker category, Police verification certificate( if required from both India & USA) and the fee I would require to pay, in addition to adding my wife as a dependent. Also wish to know if any additional documents would be required.

The immigration department's website will answer all those questions & help you further.
You need police clearance from both countries.
