It is $ 30 per document. So if you just want replacement, only apply for that and pay $ 30.

If you apply for Verification of Status along with the replacement you will pay $ 60.

VOS is of NO use to as BLS or Indian embassy won't accept that.

Make sure you pay the $ 30 with your credit card and print the paid receipt and attach it with your application. That receipt is very important and HAS to be attached with the application as that's your only proof of payment.

Keep a Xerox copy of whatever you mail to them for your own records. So you can follow up later on if there are any delays from their side.
