My mom is a new immigrant. I want to apply for her health card. On the website it indicated that the following documents are required:
1) Proof of citizenship: This one is ok, i will take COPR
2) Support of Identitiy: This one is ok, i will take Passport
3) Proof of residency: I am not sure how to prove this, she does not have any bank account and she dosent plan to get a drivers license. Any ideas?

I think it will help if you open up a bank account on her name. Look for one that does not need any minimum account balance. I guess the most easiest

Go to Motor Vehicle Department and Pay $35 and get an Ontario Photo ID.
It will help you a lot.

Also go to any Loblaws or President Choice Pavilion such as Fortinos or other centers close by and apply for a Bank Account. It is free and their services are FREE too. That will give her a DEBIT Card on the spot. Drop a few bucks in her account and you are all set.

That is for the cards with pictorial ID. Also get a Library Card for her from any Library. That is a great help too.

You are her best support. Show the Ministry of health that you are her SON and she lives with you. Give them your address at that time.

Good Luck.

Proof of residency is the key document , as you HAVE to be a resident of ON to get OHIP.

As mentioned above get the ON Photo ID card.

Or open a bank account ,
utility bills like hydro, heating, water, phone land line bill , cable TV etc.

What he said.

We did the same from our landlord while renting a basement immediately after landing.

I opened a Bank Account with RBC ... They have a seniors Rebate so the account is free. After one month you get a statement in mail which is a residence proof ... Even if you apply after 2 months of landing , your health card arrives and is active after 90 days of landing.
