My mom is a new immigrant. I want to apply for her health card. On the website it indicated that the following documents are required:
1) Proof of citizenship: This one is ok, i will take COPR
2) Support of Identitiy: This one is ok, i will take Passport
3) Proof of residency: I am not sure how to prove this, she does not have any bank account and she dosent plan to get a drivers license. Any ideas?

I think it will help if you open up a bank account on her name. Look for one that does not need any minimum account balance. I guess the most easiest

Proof of residency is the key document , as you HAVE to be a resident of ON to get OHIP.
As mentioned above get the ON Photo ID card.
Or open a bank account ,
utility bills like hydro, heating, water, phone land line bill , cable TV etc.

You are her best support. Show the Ministry of health that you are her SON and she lives with you. Give them your address at that time.

I opened a Bank Account with RBC ... They have a seniors Rebate so the account is free. After one month you get a statement in mail which is a residence proof ... Even if you apply after 2 months of landing , your health card arrives and is active after 90 days of landing.
