You normally have a Birth certificate: Check your name as listed in it.
You might have a Graduation Certificate : Check your name as listed in it.
You will also have a Passport from your home country: Check your name as listed in it.
You have Landing Papers from Canadian Government: Check the name as listed in it.
You have your PR Card issued by the Canadian Government : Check your name as listed in it.
If you want your name as listed in YOUR PR CARD, Then, Fill in the name in your application as you find in your current PR Card.
Provide them with the proof of it and the reason behind it.
Also fill in the IMM 5574E the document check list. It will tell you what all you will need to provide as proof of name and requirements there of to get your NEW PR CARD.
Please do what ever that they want. Currently it takes about 59 days or so to obtain your NEW PR Card. Please call them and verify.
