I got my PR in February 2003 and OHIP card in May 2003, presently I am out of country. Recently I have received Forms from the Ontario Ministry of Health asking me & my wife to personally bring the completed form and health card to the stated address before 9th January 2004, otherwise they may cancel the Health Card.

How I can request for the extension of the deadline and would there be any repercussions if I fail to submit it in person or on time? Does any one have any experience or thoughts?

Can you elaborate why you have been asked to fill up some new form and submit it personally to OHIP office. I personally have not heard of such a thing, though my own card was cancelled because I had stayed out of the country for 31 days out of the first 90 days and the rule allowed one to stay a max of 30 days in first 90 days.
